Tolvaptan: From SIADH-Related Hyponatraemia and Polycystic Kidney Disease to Polycystic Kidney Disease 3 (With or Without Polycystic Liver Disease)
One-Sentence Summary
Tolvaptan is a selective vasopressin V2 receptor antagonist, originally used for inappropriate ADH syndrome (SIADH), nephrogenic syndrome of inappropriate antidiuresis and polycystic kidney disease. The TxGNN model predicts it may be effective for polycystic kidney disease 3 with or without polycystic liver disease. Currently no registered clinical trials are linked to this exact indication, but 20 publications support the direction, including 2 RCTs (in ADPKD generally) and a Cochrane systematic review.

Why is This Prediction Reasonable?
Tolvaptan blocks the vasopressin V2 receptor in the kidney. In polycystic kidney disease, raised intracellular cAMP drives cyst-cell proliferation and fluid secretion. By blocking V2 receptors, tolvaptan lowers renal cAMP and slows cyst growth. Note that detailed mechanism-of-action data from DrugBank were not available for this report; the description above comes from the mechanistic analysis compiled for this prediction.
The original indications already include polycystic kidney disease, and PKD3 (GANAB-related) belongs to the same disease family with a shared cyst-formation pathway. This prediction is therefore a close extension of an existing indication rather than a wholly new one.
An important caveat: the supporting RCTs (TEMPO 3:4 and REPRISE) were conducted in ADPKD generally (PKD1/PKD2), not specifically in GANAB-mutated PKD3. Direct evidence for PKD3 is lacking.

Safety Considerations
- Hepatotoxicity: Tolvaptan in ADPKD is associated with aminotransferase and bilirubin elevations, so liver function monitoring is required (REMS-type guardrails).
- Contraindications from the knowledge graph (not label text): liver failure, cirrhosis of liver, substance abuse/dependence, active peptic ulcer disease, demyelinating disease.
- Drug Interactions (100 interactions recorded; examples):
- Major: clarithromycin
- Moderate: aprepitant, dexamethasone, empagliflozin, miconazole, rifaximin, rolapitant, naltrexone, and potassium products (citrate, bicarbonate, chloride, gluconate)
- Minor: fidaxomicin, naloxegol, prucalopride
Please also refer to the package insert for full warnings and contraindications, as label text was not available for this report.
Conclusion and Next Steps
Decision: Proceed with Guardrails
Rationale: Tolvaptan has strong RCT and systematic-review evidence in ADPKD, and PKD3 shares the same cAMP-driven cyst mechanism, making this a close extension. However, no trials specifically address GANAB-related PKD3, and hepatotoxicity requires careful control.
To proceed, the following is needed:
- Singapore package insert warnings and contraindications (not yet reviewed)
- Approved indication wording for the three Singapore licences
- Detailed mechanism-of-action data from DrugBank
- Evidence or case series specific to PKD3 (GANAB) and polycystic liver disease
- A liver function monitoring plan and patient selection criteria, taking into account the contraindicated conditions and major interactions above
